About the Author
Robert Terkeltaub, M.D., is the Chief of Rheumatology and director of the Rheumatology Training program at the VA Medical Center, San Diego, CA. In addition, Dr. Terkeltaub is a Professor of Medicine in Residence at the University of California, San Diego. N. Lawrence Edwards is a specialist in internal medicine and rheumatology, and serves as professor of medicine in the Division of Clinical Immunology at the University of Florida in Gainesville, Florida. He is also program director of the Medicine Residency Training Program and vice chairman for the Department of Medicine at the University. In addition, he is the chairman and chief executive officer for the Gout & Uric Acid Education Society

Evolution of the Understanding of Gout

This is the second edition of an important book that presents twenty-first century information about gout and hyperuricemia intended for clinicians, but useful for all students of the 2011 view of gout etiology and treatment. Its information is presented in a compact and useful manner, with text elucidated by many figures and tables, and supported by multiple references. What it presents, it presents well and comprehensively.
One of the significant evolving gout topics not covered in the book is the connection with sleep apnea. Recent publications have described the physiological etiology of gout from the hypoxemia of sleep apnea which results in cell catabolism generating excess uric acid, reduced serum pH, and acceleration of the long term deterioration of kidney function. Sleep apnea may be the principal reason that most gout flares begin during sleep. Furthermore, the recently reported life-threatening comorbidities of gout mentioned in the book -- cardiovascular disease, hypertension, kidney disease, and diabetes -- are all recognized consequences of sleep apnea.
The book's authors wisely advise, "Clinicians should always consider what the cause of hyperuricemia is in the individual patient." As new twenty-first century evidence evolves about clinically significant aspects of gout and hyperuricemia, I anticipate their coverage in future editions of this book in the same lucid manner as the presentation in this second edition.
